1. Derailed · Valuables Under the Bridge

Michael waits for Trevor at his trailer in Sandy Shores, chatting with Ron. Soon, Trevor returns from a walk with Patricia and presents Michael with a solution to return to Los Santos. He explains that a cargo train operated by Merryweather Security carries an item of immense value that will quench Madrazo's rage. Trevor instructs Michael to wait with a boat under the bridge in Raton Canyon, while he departs first to chase the train on a Sanchez motorcycle.
Spotting the cargo train north of Grapeseed, Trevor accelerates his motorcycle to ride alongside the tracks. Using a slope in the terrain as a ramp, he successfully lands the motorcycle onto the moving train. Navigating the narrow, swaying roof, he reaches the lead car, abandons his bike, and enters the engine room. He immediately subdues the engineer, takes control of the train, confirms Michael's position via radio, and pushes the train to higher speeds heading north.
At the same time, Michael pilots a small boat up the narrow waterway of Raton Canyon, positioning himself under the massive railway bridge. The cargo train, controlled by Trevor, enters a collision course with an oncoming train. Just before the impact, Trevor jumps from the engine room into the river to save his life, and the two trains collide with a deafening roar, derailing. As the cargo containers spill into the river below the bridge, Michael approaches the wreckage and attaches sticky bombs to a container door, detonating them.
Inside the container opened by the explosion, Michael finds the target—a valuable ancient fertility statue—and packs it away. Meanwhile, Merryweather armed forces, having detected the derailment and theft, arrive in high-speed boats along the river. Trevor, positioned on a small island by the riverbank, uses an assault rifle to pick off the approaching enemies. When two Merryweather helicopters appear on both sides of the canyon, Trevor focuses his firepower to shoot them down, buying Michael time to secure the cargo.
Merryweather mercenaries on the railway bridge begin sniping and firing. Trevor activates his sniper rifle's thermal scope to identify and precisely eliminate enemies on the bridge and reinforcements parachuting down. Once the area is clear, Michael starts the boat, and Trevor joins him as they speed through the canyon. During their escape, they are persistently pursued by Merryweather boats, off-road vehicles, and additional helicopters, all of which Trevor destroys with intense fire from the boat.
Reaching the Pacific Ocean, the two land on a secluded beach near Los Santos. Michael reveals the artifact, claiming it will be the perfect bribe to restore their relationship with Madrazo and return to Los Santos. Trevor, resentful that Michael is taking the spoils of his operation, attempts to seize the bag. However, when Michael proposes the Union Depository heist—a dream he has held since the Ludendorff days—Trevor relents and tosses the bag to Michael, conceding the cargo.
Trevor warns Michael that the Union Depository heist is essentially a suicide mission, but agrees after Michael convinces him that it is possible if Lester Crest and Franklin Clinton join. Trevor also decides to return Patricia to Madrazo, leaving Michael with a pointed warning that he will be watching his future actions closely. The two depart the beach in their respective vehicles, and Michael contacts Lester to negotiate with Madrazo.
2. Monkey Business · The FIB Enters the Drain

Michael, Trevor, and Franklin meet corrupt FIB agents Steve Haines and Dave Norton at the Cape Catfish pier. Steve coercively assigns them the mission to steal a lethal neurotoxin, claiming the IAA is developing it to secure more government funding. Franklin, unaware of the full scope of the operation, is tasked with waiting at the Sandy Shores airfield and leaves the scene. Michael, Steve, and Dave change into wetsuits, take a boat to the coast near Humane Labs, and begin a stealthy infiltration through the drainage pipes.
The three dive into the ocean and reach the entrance of a massive drainage tunnel connected to the lab. Michael uses an oxygen cutter, burning at over 3,000 degrees Celsius, to cut the joints of the thick iron grate blocking the tunnel. Once the grate is removed, they swim through the pipes to enter the lab. Surfacing and donning masks, the three follow Steve's instructions, using stun guns to incapacitate guards and researchers in a non-lethal manner as they carefully navigate the corridors.
Taking an elevator down to the secure area, they avoid patrolling guards and move deep into the laboratory. Michael swipes a security card, enters the control room, and threatens a researcher into opening the door to the neurotoxin storage area. The moment Michael carefully removes the neurotoxin from its container, an emergency alarm sounds, alerting the entire lab to the intruders. The stealthy infiltration ends, and armed guards and military personnel from Humane Labs swarm in with live ammunition to stop them.
Michael and the two agents fight their way through the lab corridors. Michael warns them that if he is shot, the neurotoxin could leak, as he continues to kill enemies. After breaking through heavy resistance in the animal lab and loading dock, Michael exits the building and, per Dave's instructions, places the neurotoxin into a secure refrigerated container. Trevor soon appears overhead in a military transport helicopter, the Cargobob, lowering a hook to safely extract the container.
However, as IAA response units arrive at the lab, time for escape becomes critical. Steve shoots himself in the leg to manage the situation and evade responsibility. He decides to frame it as if he were wounded while trying to stop the operation as a double agent and remains at the scene. Michael and Dave hurry out of the lab, while Steve attempts to control the situation by telling the arriving IAA agents he is a special FIB agent, though he faces immediate detention.
Trevor pilots the Cargobob toward the Sandy Shores airfield, while Michael informs him via radio of the farce Steve staged with his self-inflicted wound. Upon arriving at the airfield, Trevor lowers the container onto a truck trailer and lands the helicopter. Franklin, who was waiting, approaches to ask what happened, but Michael, hiding the nature of the dangerous cargo, expresses his anger to Dave, vowing never to be involved in such things again. Dave leaves with the neurotoxin, and Michael feels bitter about the hypocrisy of state agencies and his own status as a mere tool.
3. The Return of Patricia · The Kidnapping Ends and Relationships Remain

With Michael delivering the fertility statue secured from the Merryweather train to Madrazo via Lester, the negotiation to resolve the conflict with the Madrazo cartel is successful. However, the core condition of this agreement is the safe return of Patricia Madrazo, whom Trevor had kidnapped. Trevor drives Patricia from his trailer to the Madrazo mansion in Los Santos. Inside the car, Trevor struggles with tears, feeling a sense of loss as Patricia leaves him.
Patricia comforts Trevor, telling him he is a good man with deep inner wounds, but draws a line, stating she has a duty to return to her husband. Upon arriving at the mansion, Martin Madrazo, with his ear bandaged, greets them in a state of extreme terror. Trevor approaches Madrazo and issues a stern warning never to mistreat his wife again. Crushed by Trevor's threat, Madrazo humiliatingly nods, swearing to treat Patricia well.
Despite being disappointed by her husband's violent nature and infidelity, Patricia returns to Madrazo's side to maintain her place as a wife. Trevor suppresses the emotions and attachments he cannot control and turns away. While both Madrazo's violent dominance and Trevor's unilateral kidnapping were unjustifiable acts of coercion, Patricia continues to contact Trevor afterward, leaving a strange bond between the two.
By safely returning Patricia, Michael and Trevor finally gain the justification to be free from the threats of the Madrazo cartel and return to Los Santos. Michael plans to end his life as a fugitive in the desert and head to Richards Majestic Studios to help film producer Solomon Richards. Trevor also moves to the next stage of establishing a new base in Los Santos and preparing for the Union Depository heist.
However, the reconciliation with Madrazo only removed an immediate external enemy; it did not resolve the fundamental conflict between Michael and Trevor. The truth behind Brad's death and Michael's faked death in Ludendorff remains like a ticking time bomb. While the two are temporarily cooperating for the common goal of a massive bank heist, their mutual distrust and hidden secrets leave them in a precarious state as they face a new phase in Los Santos.
4. Hang Ten · From Floyd's House to the Club

After dropping Patricia off at the Madrazo mansion, Trevor heads to Floyd's apartment in Vespucci Beach. There, Floyd's girlfriend, Debra, who had just returned from a business trip, was waiting. Seeing her home in shambles, Debra is enraged and screams at Trevor to get out immediately. Trevor refuses to leave, worsening the situation by mocking Debra, suggesting she cheated on Floyd while she was away.
Debra, reaching the peak of her anger, yells at Floyd to get out as well, and eventually pulls a pistol from a drawer to aim at Trevor. Provoked, Floyd also picks up a kitchen knife to confront Trevor, and the tension in the room reaches its breaking point. Trevor looks at the two armed people, coldly stating they are not good people, and adopts a threatening posture. Immediately after, the screen fades to black, and the specific process of the tragedy that occurred inside the apartment is omitted.
A moment later, Trevor walks out of the apartment, gasping for air, covered in blood on his face and clothes. While it is not explicitly shown who attacked first or the order in which the killings occurred, it is implied that Floyd and Debra met a gruesome death at Trevor's hands. Trevor sits on the stairs as if nothing happened until he encounters Wade Hebert, who had come to visit his cousin, Floyd.
To prevent Wade from entering the apartment and witnessing the horrific scene, Trevor urgently turns him away and suggests they go to a new house. Trevor drives Wade to the Vanilla Unicorn strip club in downtown Los Santos. Upon arrival, Trevor calls over two strippers, telling them to keep Wade entertained for a long time, completely distracting him.
While Wade is distracted, Trevor storms into the office of the club manager, Leon. He informs Leon that he is the new partner, kills him off-screen, and stuffs the body into the office refrigerator. Having eliminated the manager and taken full control of the Vanilla Unicorn, Trevor makes it his new Los Santos operations headquarters and safehouse. The apartment at Vespucci Beach remains a horrific crime scene, no longer accessible.
Through a bloody slaughter, Trevor brutally cleared the obstacles in his path and secured a new base. He controls the club's operations in his own way and is ready to join the Union Depository heist preparations being conducted by Michael and Lester in earnest. The tragic deaths of Floyd and Debra once again prove Trevor's uncontrollable violence and signal the prelude to greater crimes to be committed in the dark underbelly of Los Santos.
