Key information
A recent update to Old School RuneScape intended to introduce a PvP tutor NPC inadvertently triggered a severe bug. This technical issue caused bosses to double-kill players, resulting in the permanent loss of valuable gear that should have been retrievable.
Under normal mechanics, players dying in boss encounters typically have their items moved to a retrieval chest. However, the bug kept characters inside the arena after their initial death, leading to an immediate second death that wiped the contents of the first retrieval inventory.
Jagex responded by taking worlds offline to implement a fix. The developer confirmed the issue and is currently working to restore the lost items for affected players without resorting to a full game rollback.
The incident has prompted community debate regarding the harshness of the game's death penalties. Jagex acknowledged these concerns, stating that refining the item retrieval system is now a priority for their quality-of-life team.
